Potomac Home Health Care (, a non-profit joint venture of Sibley Memorial Hospital and Suburban Hospital, is a full service, medicare-certified home health agency which provides a comprehensive range of home care services in Maryland and Washington, DC. Using a team approach, we make every attempt to recognize and meet the needs of homebound patients. 
Potomac Home Health Care meets the needs of the community by providing cost efficient, quality home health care and patient education services to help patients achieve maximum independence and an enhanced quality of life.
Our program stresses personalized care, tailored to the needs of each patient. We work closely with the physician, the patient, and family members to plan, review progress, and make adjustment to the plan of care. This approach results in a better treatment plan and greater understanding and support for patients and family members. Our program has the unique feature of offering the results of two full-service hospitals.

Potomac Home Health Care Services

Skilled Nursing
A registered nurse performs a comprehensive nursing assessment, develops a plan of care, and coordinates care with the physician and other team members. The nurse performs nursing procedures and assists patients and caregivers to become independent in care by instructing them in the disease process, medications, and symptom management.

Infusion Therapy
Our team of professionals provide comprehensive infusion services including hydration, antibiotics, chemotherapy, total parenteral nutrition, pain management, blood products, catheter care, and enteral nutrition. Our infusion nurses promote an environment conducive to the successful completion of therapy and the safe delivery of care.
A registered clinical dietitian assesses the patient's nutritional needs based on medical diagnosis and current habits. The dietitian then teaches the patient or caregiver to achieve the defined nutritional goals.
Medical Social Work
Licensed clinical social workers (LCSW and MSW) participate in the development and implementation of the plan of care by assessing social factors which impede recovery and coordinate use of community resources and special support groups. Social workers also counsel patient or family members if there is a need related to loss of health and function and/or a need for psychiatric services.
Physical Therapy
A licensed physical therapist assesses the patient's ability to function in the home by evaluating strength, range of motion, endurance, coordination, balance, pain, posture, and mobility. The therapist develops and implements a rehabilitation program which will help the patient maximize total function.
Occupational Therapy
A licensed occupational therapist assesses the patient's ability to perform activities of daily living and fine motor functions, the need for adaptive equipment in the home, and the patient's cognitive and perceptual function. The therapist develops a program geared to maximizing safety and independence.
Speech-Language Therapy
A licensed speech-language pathologist assesses and treats speech, language, and swallowing disorders. Specific treatment programs are developed for aphasia, voice disorders, apraxia, dyspraxia, dysphagia, speech, and language in order to maximize the patient's communication and/or swallowing skills.
Home Care Aide
Home care aides provide or assist patients with such personal care as bathing, dressing, oral hygiene, meal preparation, feeding, walking, and exercise. These services are designed to help patients regain maximum independence in providing self care.

Payment Plans

Home health care services are covered by Medicare, Medicaid, Blue Cross, CHAMPUS, and most commercial insurance policies. We also honor private-pay. To be eligible, patients must:
  • Be confined to the home
  • Require skilled nursing or therapeutic services on an intermittent basis
  • Have a home health plan established by a physician 


Contributions to Potomac Home Health Care are used to provide services to individuals who are unable to afford home health care.
